By the end of this session participants should be able to: 1. Develop anti-oppressive and trauma-informed approaches to providing primary care for patients experiencing homelessness. 2. Adapt screening, diagnosis, and management approaches for several common health conditions to the experience of homelessness. 3. Implement strategies in clinics to facilitate care coordination for patients experiencing homelessness address their unique social determinants of health.
