Describe the complex clinical, developmental, and systemic challenges of transition from child and adolescent mental health services (CAMHS) to adult mental health services (AMHS), drawing on longitudinal, multi-country evidence from the MILESTONE studies. 2. Equip clinicians, researchers, and service planners with evidence-based tools and validated measures (e.g., Transition Readiness and Appropriateness Measure (TRAM), Transition Related Outcome Measure (TROM), On Your Own Feet - Transition Experiences Scale (OYOF-TES)) to enhance transition readiness assessment, continuity of care, and outcome evaluation. 3. Synthesize key insights from the extensive MILESTONE research outputs, including predictors of successful transition, diagnostic and pharmacological determinants, and youth-informed experiential data, to inform best-practice and future policy development across Europe.
