CANCELLATION POLICY 
Refunds less $75 administrative charge will be issued to cancellations receive in writing by May 10, 2024. No refunds will be issued for cancellations received after that date. All refunds will be processed after the event.

 
LEARNERS TERMS AND CONDITIONS

STATEMENT OF OCPD PRIVACY OBLIGATIONS TO LEARNERS AND SPEAKERS
OCPD is committed to the protection of all personal information collected and used in the operation and management of its programs. All personal information provided to OCPD will be protected in accordance with uOttawa Policy 90 and the applicable privacy legislation, including the Freedom of Information and Protection of Privacy Act. OCPD and the 2024 Precision Child Youth Mental Health will not provide learner and speaker information (e.g. disclosure agreements, communications with speakers) to any third parties such as external organizations, sponsors or other industry concerns. OCPD does provide the Ottawa Hospital Academic Medical Organization with lists of members who speak for OCPD so they may qualify for TOHAMO recognition. Information collected by OCPD may be amended or deleted by the learner or speaker provided reasonable rationale and identification are provided. Learners and speakers may opt out of future contact either through the registration system or contacting us directly at: cpd@uottawa.ca

 
POLICY ON PATIENT PRIVACY
As our programs deal with clinical content, cases are frequently used in presentations. OCPD follows Ontario’s Personal Health Information Protection Act, 2004 (PHIPA) which prohibits the disclosure of a patient’s healthcare information outside of a patient’s circle of care. While the Act permits use of personal health information for “educating agents to provide health care” {37(1e)}, OCPD directs speakers to ensure that any case material does not provide any patient identifiers.

 
USE OF PHOTOGRAPHS AND RECORDINGS
I UNDERSTAND that the photographs, videos or audio recordings of me may be circulated widely and that, if posted on the University of Ottawa’s website, in its online searchable image bank or on any other websites, they will be available to the public. I also understand that the University has no control over, and is not responsible for, the use or misuse of materials available on its website, including any photographs, videos or audio recordings of me. FOR THE PURPOSE STATED ABOVE, I CONSENT to be photographed, video recorded and audio recorded by the University of Ottawa or its authorized representatives. I ALSO GIVE MY PERMISSION to the University of Ottawa, to its representatives and to users of the University’s online searchable image bank to use, reproduce, publish, transmit, distribute, broadcast and display photographs, videos or audio recordings that contain my image or voice, with or without my name associated with them. In any current or future University of Ottawa material, publications, multimedia productions, video, displays, advertisements and on the University’s website, social media website and other current or future media, without further notice to me or without my approval of the finished photographs, videos or audio recordings.

SCIENTIFIC BALANCE AND USE OF BEST EVIDENCE:

OCPD requires that all speakers follow the standards adopted by the Royal College of Physicians and Surgeons of Canada and the College of Family Physicians of Canada which states that speakers must respect the principles of scientific integrity, objectivity and balance which must not be affected by any commercial influence.  The description of diagnostic or therapeutic options must use generic names, avoid brand names and provide balance in providing other alternatives.

 
COPYRIGHT:
You must ensure that your Speaker Presentation(s) and use of handouts respect copyright laws. You must also respect the following speaker guidelines:  https://copyright.uottawa.ca/instructors.. You warrant that your Speaker Presentation(s) and any material you use in your presentation are your original work and/or that the content does not violate or infringe on any copyright, trademark, trade name, moral rights or any other intellectual property right of a third party and /or that you have or will obtain the necessary permissions for any content owned by third parties.  You agree to compensate OCPD from and against any loss, costs, expenses or liability incurred by OCPD arising out of or in any way connected to your breach of this warranty.  You also give to OCPD a non-exclusive, royalty-free permission to print, reproduce, publish, translate, adapt, transmit and distribute your Speaker Presentation(s) and /or handouts throughout the world, by any means, and in any format or medium whether existing or which may come into existence in the future, in perpetuity and for OCPD purposes only. This permission includes the ability to store the Speaker Presentation(s) and/or materials in databases, archives, searchable collections of resources and analogous entities in whatever format or medium they exist.  Other than the one-time payment described in this Speaker Agreement, no further compensation, royalty or other monies are owed or will be owing to you by OCPD in connection with your presentation, the preparation or use of your Speaker Presentation(s) and/or handouts.

AUDIT OF EDUCATION PROGRAMS: 
OCPD is committed to ensuring that the work of the Scientific Planning Committee results in an excellent education program.  Using a validated audit tool, OCPD will assess its programs to evaluate:

a.          Appropriate disclosure of Conflicts of Interest;

b.          Delivery of intended content to fulfill the developed learning objectives;

c.          The use of best evidence in presentations/workshops;

d.          The absence of bias including absence of industry logos, product promotion;

e.          Proper disclosure of any off-label use of medications or devices;

f.           Adequate time devoted to audience discussion and interaction either through case or table discussions, Q+A.

g.          To maintain confidentiality of speaker evaluations:  overall course evaluations may be shared with program committee members whereas individual speaker evaluations should be considered private for use only at the discretion of program committee chairs and not disseminated unless permission is granted by the speaker;

Members of the Faculty of Medicine have a commitment to uphold the highest standards of ethical and professional behaviour consistent with the values of the University of Ottawa. OCPD requires speakers to ensure that their contribution to OCPD activities are consistent with the uOttawa Faculty of Medicine
