By the end of this session, participants will be able to:
Outline the unique challenges in navigating health care system in the absence of Primary Care Provider. (i.e. Facilitating referrals, obtaining prescriptions for ongoing pharmacological care, accessing care and services)
Explain the influence of the Social Determinants of Health and how this impacts non-attached clients, (i.e. newcomers in the context of language and cultural differences, unhoused persons, poverty and those living with disordered mental health);
Summarize how to support pregnant clients to prepare for birth, decreasing social isolation and optimizing environment for success.
