OntarioMD (OMD) - Insights4Care ProgramOMD's Insights4Care (i4C) Program helps you understand the importance of patient data and quality improvement initiatives for your practice and patient care. Our i4C Dashboard tool unlocks the value of your real-time EMR data to provide population health insights that can lead to better patient outcomes. The Dashboard provides visual representations of high-value clinical indicators across patient rosters, at-a-glance and on one screen. The Dashboard’s use is growing and is already being used by more than 1,500 physicians across Ontario. The i4C Advisory Service is a team of highly experienced QI staff and EMR experts at OMD who are delivering parts of the Essentials for Practice Improvement for Ontario Physicians course program. They will guide you during and after the course to apply the knowledge you’ve gained to QI initiatives for your practice. Their support is complimentary to physicians who use an OMD-certified EMR. POPLAR                               

The Primary Care Ontario Practice-based Learning and Research Network is a provincial not-for-profit organization involving a partnership between researchers, clinicians, and policy makers that aims to help advance knowledge in primary care and support quality improvement efforts. The network includes over 1,000 Primary Care providers who contribute de-identified EMR records for their 1.8 million patients to a secure database which allows POPLAR to study primary care data and to provide each of its over 1,000 participants regular reports on a comprehensive set of quality indicators for their practice.  The reports showcase the results for their practice in comparison to the global regional metrics and are useful in helping practices identify areas for improvement and monitor progress over time.
