1:15 - 2:45PM | QI Framework, Writing SMART Goals and Using Tools Topics: Review the Model for Improvement – where are we now? What is a SMART goal + AIM statement (Note: CPSO requires 2 SMART goals for QI/QA) What tools can I use to help me identify an area for improvement: Identify root cause, the 5 WHYS; select area to address using Pareto. What measurements can I use to follow to see if a change leads to an improvement: outcome, balancing and process measures. What are the elements of a QI plan? |

Implementation strategies and how to make your change sustainable Topics: Review the Model for Improvement – where we are now? What is the PDSA cycle? More than one change may have to be tested: not every change is an improvement but every improvement requires change Once a change seems to work, how do I implement it in my practice? Any resources I can use? How do I sustain the change? How can I identify other opportunities for change? |
